These are the statutory Texas forms for Medical and Durable Powers of Attorney, HIPAA Release and Directive to Physicians. As is the case with any generic form, great care must be taken to ensure that the person signing understands the effect of doing so. CAUTION: Powers of Attorney grant broad and sweeping authority to the person(s) designated in them. A person to whom you grant such powers will have the ability and authority to conduct significant business transactions in your behalf, including the sale and liquidation of your assets.
